ÁùºÏ±¦µä’s Family to Family Health Information Center for families who have children & youth with special health care needs.
Caring for children with special needs and disabilities can be overwhelming, exhausting and complex. Often families and caregivers struggle to find the resources and services their child and family may need.
The Family Navigation Network (FNN) is a statewide project of the ÁùºÏ±¦µä Center for Excellence in Disabilities providing information, support, training and resources at no cost to assist families caring for children and youth with special needs. We are staffed by parents and family members of children with special health care needs and disabilities, so we know the journey.
We promote family-centered care and family and professional collaboration at all levels of health care. The FNN also has a strong commitment to promote and support the needs of families from racially, culturally, and linguistically diverse communities.

We provide emotional support for families of individuals of all ages with disabilities and/or special health care needs.
We help families who are struggling with Medicaid coverage for their loved ones.
We offer a monthly workshop series on a variety of topics relating to youth as they transition from pediatric to adult services.
