Graduate faculty forms
The Graduate School uses DocuSign Powerforms for all forms that require signatures, and web forms (Formstack) for all forms that do not require signatures.
We do not accept PDF or printed submissions. If a printed and/or handwritten form is submitted, it will be returned and delay your approval process.
DocuSign automates the routing and collection of digital signatures. For each form linked below, a pdf version is provided solely as an instructional template for preparing the DocuSign version, not as an alternative form of submission. If you are new to using Powerforms, please read through the DocuSign instructions to understand how to initiate and complete your form. This form should be used when requesting an exception to a Graduate School policy related to admissions only. For other exemption requests, use the General Exception to Policy Request below.

All doctoral programs that require a dissertation are now required to hold both a public and a private defense of the dissertation. The public defense, which precedes the private defense, must be announced in the University events calendar.
An Employment Overload Request form is used when a department wishes to increase the number of hours a student is working.

These grants provide travel funding resources for faculty and are processed in cycles throughout the year.

Student Learning Outcomes are used for accreditation purposes and help individual programs define and measure student success. These forms help track program efficacy over time, while also providing an opportunity for advisors to provide input on the SLOs themselves and how they might be better defined or adapted to current needs within their respective fields.

Applicants for Master’s degree programs with an overall undergraduate GPA between 2.3 and 2.74 may be admitted with a Prescribed Program Request Form.

Requests to transfer between programs within the same college must be approved by current and new Graduate Program Directors and the Graduate School.

If a graduate assistant fails to meet pertinent work standards, the Recommendation for Termination of Graduate Assistantship Form must be used to terminate a graduate assistantship appointment and the attendant stipend prior to the end of the GA’s appointed term of service.
